PREA Performance Questionnaires
Baseline Questionnaire - This questionnaire is to be completed ONCE at the beginning of the grant during the first reporting period. Answers to these questions should reflect the agency’s status before conducting any grant activities (i.e., business as usual before PREA program funds). The intent of this questionnaire is to gather descriptive data about the agency that is conducting PREA-related activities as part of the grant.
General Questionnaire - This questionnaire is to be completed by January 15, April 15, July 15, and October 15 each year the award is open. All recipients receiving funding under the PREA program must submit quarterly performance metrics defined by the U.S. Department of Justice. The performance achievements for this program are established to show the impact and value of the program and indicate how grant activities meet the PREA objectives.
Post-Award Questionnaire - This questionnaire is to be completed ONCE at the end of the grant period (i.e., before closeout). Answers to these questions should reflect the agency’s status after conducting grant activities. The intent of this section is to gather descriptive data about the agency that is conducting PREA-related activities as part of the grant.
Tiered Subrecipients
The following forms apply to recipients who have entered into a pass-through entity/tiered subrecipient relationship with a third-party. Additional forms may be required based on the tiered subaward project. Please contact your OCJG grant manager with any questions regarding what additional forms may be required.
