State Program Forms
The following forms are for
State Financial Assistance (SFA) programs as appropriated by the Florida Legislature to local criminal justice agencies, institutions of higher education, and non-profits.
NOTE: SFA forms are program specific and cannot be used for submission under another program.
General Appropriation Act (GAA) Projects
The following forms apply to the GAA program.

MUST be signed by the Chief Official of the Recipient Agency (i.e. Sheriff, Chief of Police, etc.).
