Tiered Subrecipient Forms
The following forms apply to recipients who have entered into a pass-through entity/tiered subrecipient relationship with a third-party. Additional forms may be required based on the project. Please contact your Criminal Justice Grants (CJG) grant manager with any questions regarding what additional forms may be required. Performance Questionnaires
The following forms are required for performance reporting. Please contact your CJG performance & monitoring grant manager with any questions regarding performance reporting requirements.
Standard Conditions
Recipients must comply with the Standard Conditions of their award agreement upon signing acceptance of the award. The following conditions apply to all federal programs by federal fiscal year. Please contact your CJG grant manager with any questions regarding the Standard Conditions.
