Criminal Justice Grants (CJG) uses numerous forms and certifications for the administration of awards. Some forms are program specific; whereas others will apply regardless of which program you receive funding from. To access these forms, please select the appropriate link located below.

DCRA	Death in Custody Reporting Act (DCRA) Required for all state and local law enforcement agencies, as well as, correctional agencies per Pub. L. No. 113-242.
Safe Policing	Safe Policing and the President's EO 13929 Required for all state and local law enforcement agencies seeking DOJ discretionary grant funding per Executive Order No. 13929.
Federal Program	Federal Program Forms Program forms for federal grants awarded through the U.S. Department of Justice (DOJ), Office of Justice Programs (OJP). Federal programs include: JAG, NARIP, NCHIP, PREA, PSN, and RSAT.
State Program	State Program Forms Program forms for State Financial Assistance (SFA) programs as appropriated by the Florida Legislature in statute or through the General Appropriations Act (GAA).

Florida Department of Law Enforcement Priorities

FDLE is composed of five areas: Executive Direction and Business Support, Criminal Investigations and Forensic Science, Criminal Justice Information, Criminal Justice Professionalism and Florida Capitol Police. FDLE’s duties, responsibilities and procedures are mandated through Chapter 943, FS, and Chapter 11, FAC.
