CJSTC Form 14 - Breath Testing Course Performance Report (Rev. 3/2013)
CJSTC-71 Instructor Certification Application, CJSTC-81 Instructor Competency Checklist, and CJSTC-84 Instructor Compliance Application are all kept by the Criminal Justice Standards and Training Commission

Training centers should submit all course documentation to ATP. Delivery methods include:

Please alphabetize documents based on the course roster (CJSTC-67) *We no longer require the Breath Test Training Report as the form is obsolete. Participants' proficiencies are required for your records, however do not need to be submitted in order to process the class.
